Nursing Care Services’ Home Care Specialists provide support services to your loved ones by assisting them in maintaining their independence and quality of life in the comfort of their own home or life care community.
Nursing Care Services is Home Care and Home Health licensed, as well as an approved HCIS provider, and an approved provider of both the Bucks and Montgomery counties Departments of Aging.
Working with you, your physician, and a Home Care Specialist we’ll develop a program of home care based specifically on your needs and the needs of your loved ones. Our program determines the type of services required; the appropriate home health team members who will provide these services; as well as how often each client will be seen by participating team members. The program also includes required medications, needed medical equipment, supplies and treatments, as well as each client’s personal goals for improvement.
In addition, our individualized programs are designed to provide peace of mind to our client’s family by ensuring that the quality of life of their loved one has not changed, even though some of the circumstances in regard to their health have changed. Our goal is to ensure that all of our clients’ loved ones age gracefully and safely with dignity and respect.
If your loved one is living in a life care community, we will take care of them in their apartment so that they don’t have to go into a skilled nursing or personal care facility. If someone you love is being discharged from the hospital and requires additional home care, our staff will be there to provide the necessary care and alter their care as needed.
Our services can be utilized on a short-term or long-term basis, ranging from just a few hours to continued daily care for many years. No matter the situation, Nursing Care Services is there to assist our clients and their families through any type of heath crisis, medical need, and/or personal care service (including hospice support) as we are dedicated to never taking on any situation that we cannot successfully complete.
